It is our practice that all prospective candidates consent to undergo a set of pre-employment checks. Please note if a criminal history check reveals undeclared criminal convictions and if you fail any of the pre-employment checks, your employment may be terminated.

Please list the details of two referees who are your two most recent managers. Please note they will not be contacted without your permission.

1. I declare that the information supplied by me is given voluntarily and is to the best of my knowledge, true, accurate, and complete in all respects. I acknowledge that I will not hold Engagement Partners responsible for any omission or misstatements that I have made in the information provided.

2. I understand that all information provided about me to you, including my resume, references, and any tests will be held by Engagement Partners for the purpose of evaluating my qualifications, experience, and suitability for permanent employment, temporary or contract positions with any potential employer. 

3. I authorise Engagement Partners to confidentially disclose any information I have provided them to any clients I have been presented to interview with, without further authorisation required.

4. I authorize Engagement Partners to undertake an online search to verify that all the information I have provided is true and correct.

5. I understand that if I withhold relevant information or supply false or misleading information about myself that my application may not be further considered. I also understand that my employment or contract position may be terminated if, after investigation, an employer or client discovers that any information which I have provided is false or misleading.

6. I understand that I am entitled to have access to relevant information retained by your organisation (except for any exemption provided under the Privacy Act 1993 such as evaluative material) and to request correction of the information and/or request that there be attached to the information a statement relating to the fact that I have requested a correction.

7. I authorise you to retain any information about me until I advise you in writing that I no longer wish to seek employment or contract opportunities through you, in which case you will update my records accordingly. I understand that details forwarded to clients or evaluative information, e.g., assessments, psychological reports are outside the boundary of this request.

8. I authorise Engagement Partners to approach the referees noted on this form and any other referees nominated by me, with regard to my past employment performance.

9. In the event that I undertake Engagement Partners Assessments I confirm that I alone have completed the assessments and no other parties have been involved.

10. By authorising Engagement Partners to send me email communications I understand that this could include anything from general email correspondence to email newsletters, promotions, information, notifications and all other email related communications in accordance with The Unsolicited Electronic Messages Act 2007.
